Chris Austin

Chris Austin is a Social Worker by background, and has worked in various geriatric arenas most of her professional career. She is a Certified Social Work Case Manager through the National Association of Social Workers, and is an Advanced Aging Life Care Professional™ through the Aging Life Care Association ( Chris resides in Lee County.

Dawn Moore

Dawn Moore RN,CMC has lived in Lee County most of her life. She has worked as a Geriatric Care Manager through LMHS Senior Care Choices Program for the past 25 years. Dawn serves as the Unit Leader for Aging Life Care Association™ SWFL unit, Advisory Council chair for the Alvin Dubin Alzheimer’s Resource Center, and is an active member of Southwest Florida Coalition for Optimal Behavioral Health and Aging. She also serves on the planning committee for the SWFL Conference on Aging. Dawn has three wonderful children and two beautiful granddaughters with another grandchild on the way.

Cindy Auskamp R.N., B.S.N., C.M.C

With over 30 years of experience in nursing, Cindy is certain that this is her calling. She graduated with a Bachelor’s Degree in Nursing from the College of Mount St. Joseph, in Cincinnati Ohio. In 2017, she will be receiving her Master’s Degree in Science. Cindy has practiced nursing in a variety of settings over the years, to include ICU, Neurosurgery, Cardiology, Occupational, Employee Health, as well as Home Health. As a Certified Geriatric Care Manager for the past 5 years, Cindy considers it both an honor and a privilege to work with clients as they age. Assisting clients and their families in making difficult decisions and graceful transitions is her primary goal
