Lee Health Home Health awarded Heart Failure Disease Program Certification under the Community Health Accreditation Partner, Inc. (CHAP) Disease Program Standards

Press Releases

(Fort Myers, Fla. – May 30, 2023) --- Lee Health Home Health has received a Heart Failure Disease Program distinction under the Community Health Accreditation Partner, Inc. (CHAP) Disease Program Standards.

The certification demonstrates that Lee Health Home Health meets the industry’s highest nationally recognized standards for managing heart failure. The rigorous program evaluation focuses on program structure and function, quality metrics, advanced education for staff and demonstrated self-management tools for patients.

The CHAP Disease Program Certification provides standards and guidance on treating heart failure that has been written and vetted by clinical industry experts. It enables clinicians to give home health care patients the tools, knowledge, specialized care and confidence to live with heart failure as comfortably as possible without needing to be readmitted to the hospital.

“We couldn’t have achieved this award without our team’s dedication and commitment to improvement. We are looking forward to continuing to provide the Southwest Florida community with passionate home health care while always striving to meet the highest industry standards,” said system director/administrator for Lee Health Home Health, Jonathan Hollander, RN, MBA.

Heart failure is when the heart isn’t pumping as well as it should be. Living with heart failure means the heart has trouble supplying cells with enough blood. This can lead to fatigue, excessive coughing and shortness of breath. Even everyday tasks like carrying groceries inside can become very difficult.

“By achieving CHAP Disease Program Certification, Lee Health Home Health has shown a commitment to excellence. We are excited to deepen our partnership with Lee Health Home Health by offering support in its commitment to providing quality care and continuous improvement,” said CHAP President and CEO, Nahan DeGodt.

Lee Health Home Health provides a variety of services, including nursing, medication management, wound care, patient and caregiver education, intravenous therapy, nutrition therapy, physical therapy, occupational therapy, speech therapy, medical social work, monitoring of serious illnesses and home health aide services to patients and their families in Lee County.

Whether a patient is learning to adapt to an unexpected health issue and needs support  recovering from a surgery or dealing with a chronic illness, Lee Health Home Health utilizes the latest mobile health technology to bring therapists, registered nurses, medical social workers and home health aides directly to the patient in their own home, where they feel the most comfortable.

Community Health Accreditation Partner, Inc. is an independent, not-for-profit, accrediting body for community-based health care organizations. Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. The organization is the oldest national, community-based accrediting body with more than 9,000 agencies currently accredited nationwide.
