Lee Health Home Health awarded Heart Failure Disease Program Certification under the Community Health Accreditation Partner, Inc. (CHAP) Disease Program Standards

(Fort Myers, Fla. – May 30, 2023) --- Lee Health Home Health has received a Heart Failure Disease Program distinction under the Community Health Accreditation Partner, Inc. (CHAP) Disease Program Standards.

The certification demonstrates that Lee Health Home Health meets the industry’s highest nationally recognized standards for managing heart failure. The rigorous program evaluation focuses on program structure and function, quality metrics, advanced education for staff and demonstrated self-management tools for patients.

The CHAP Disease Program Certification provides standards and guidance on treating heart failure that has been written and vetted by clinical industry experts. It enables clinicians to give home health care patients the tools, knowledge, specialized care and confidence to live with heart failure as comfortably as possible without needing to be readmitted to the hospital.

Heart failure is when the heart isn’t pumping as well as it should be. Living with heart failure means the heart has trouble supplying cells with enough blood. This can lead to fatigue, excessive coughing and shortness of breath. Even everyday tasks like carrying groceries inside can become very difficult.

Community Health Accreditation Partner, Inc. is an independent, not-for-profit, accrediting body for community-based health care organizations. Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. The organization is the oldest national, community-based accrediting body with more than 9,000 agencies currently accredited nationwide.
