Frequently Asked Questions about Hospital at Home Program

Hospital at Home enables patients to receive acute-level care in the comfort of their homes through in-person and technology-support care delivery. Patients can be admitted in two ways: redirect or re-bed. Potential patient admissions to a traditional hospital can be redirected from the emergency department and admitted into Hosptial at Home. Also, patients admitted to the traditional hospital, can be rebedded to complete acute-level treatment in their home. The patient's status is inpatient, even though the patient is in their home

Hospital at Home is for patients who need hospital-level care, but prefer to receive that care in their own home. The only difference is where the care is provided. Hospital at Home is not recommended for everyone. A physician will decide if you are able to receive care outside the hospital campus based on your needs.

Hospital at Home staff are monitoring your condition 24/7. A registered nurse will evaluate you and determine if it is safe to remain in your home. If you are experiencing an emergency, such as a fire or some other threat to your safety, call 911.

The Hospital at Home clinical team will evaluate your medical history and certain elements about your home
setting. Based on this evaluation, the clinical team and your physician will determine if your home is a fitting alternative to a traditional hospital admission.

No, the technology provided is hassle-free and included in the program’s plan. The technology is simple and our staff will deliver, install, demonstrate, and explain how to use it.

How Lee Health Delivers Home Based Hospital Care

The program leverages all available resources within our health system with the latest technology to provide care from the comfort of your home. Once you decide to participate in the Hospital at Home program, our team will:

1. Meet with you to discuss your care needs and the tools we will use to ensure your wellbeing and comfort.

2. Help transport you home with the supplies and medications specific to your condition.

3. Arrive at your home to set up the equipment our team will use for communication and monitoring of your care.

After you are discharged from the program, we will send a report of your care and test results to your primary care provider.

Sample Schedule of In-Home Nurse Visits

Day of Admission

  • Initial in-home nurse visit with comprehensive assessment.

  • In-home visit (twice daily) with a Lee Health nurse.

  • Virtual call with the program team to answer any questions and discuss additional resources.

Day 3

  • In-home visit (twice daily) with a Lee Health nurse.
  • Virtual visit with physician.
