- Virtual Care Doctors Visits: Routine and as-needed video visits from a doctor or nurse every day.
- 24/7 Remote Vital Signs Monitoring: Monitoring for temperature, pulse regularity, and other important indicators of health status.

- Medications and Treatment Management: Medications, IV infusions, testing, imaging, and therapy brought to your home as needed.

How Lee Health Delivers Home Based Hospital Care
The program leverages all available resources within our health system with the latest technology to provide care from the comfort of your home. Once you decide to participate in the Hospital at Home program, our team will:
1. Meet with you to discuss your care needs and the tools we will use to ensure your wellbeing and comfort.
2. Help transport you home with the supplies and medications specific to your condition.
3. Arrive at your home to set up the equipment our team will use for communication and monitoring of your care.
After you are discharged from the program, we will send a report of your care and test results to your primary care provider.
Sample Schedule of In-Home Nurse Visits
This schedule is a sample and for demonstrative purposes. Your Hospital at Home schedule may differ from the example below:
Day of Admission
- Initial in-home nurse visit with comprehensive assessment.
- Virtual visit with physician.
- IV therapy and lab testing (if needed).
- Meal delivery.
Day 2
- In-home nurse visit (twice daily) with a Lee Health nurse.
- Virtual visit by physician.
- IV therapy, x-ray imaging, and lab testing (if needed).
- Virtual check-in with the program team.
- Meal delivery.
Day 1
- In-home visit (twice daily) with a Lee Health nurse.
- Virtual visit with physician.
- IV therapy and lab testing (if needed).
- Virtual call with the program team to answer any questions and discuss additional resources.
- Meal delivery.
Day 3
- In-home visit (twice daily) with a Lee Health nurse.
- Virtual visit with physician.
- IV therapy and lab testing (if needed).
- Virtual check-in with program team.
- Meal delivery.
