What is a meniscus tear?

The meniscus is a C-shaped piece of cartilage in the knee that acts as a cushion between the thighbone (femur) and shinbone (tibia). Each knee has two menisci: the medial meniscus on the inside and the lateral meniscus on the outside. They help absorb shock, distribute weight, and keep the knee stable during movement. A meniscus tear can happen from sudden twisting motions, like during sports, or from wear and tear as you age. When torn, it can make the knee less stable and cause pain, swelling, and limited mobility.

Types of Meniscus Tears:

  • Radial Tear: Extends outward from the center of the meniscus.
  • Horizontal Tear: Splits the cartilage into upper and lower sections.
  • Bucket Handle Tear: A displaced segment creates a "handle" effect within the joint.
  • Degenerative Tear: Common in older adults due to cartilage weakening over time.

Symptoms and Signs of a Meniscus Tear

Meniscus tears can present with a variety of symptoms, some of which may overlap with other knee conditions. Understanding these signs can help in recognizing the injury early and seeking appropriate treatment. Below is an expanded list of symptoms commonly associated with a meniscus tear:

Localized Pain: Often felt along the knee joint line, particularly during twisting or squatting movements.

Chronic Ache: In degenerative tears, the pain may persist as a dull ache that worsens over time.

Activity-Triggered Pain: Discomfort often increases with activities like climbing stairs, kneeling, or rotating the knee.

Swelling: Typically develops within 24-48 hours of injury and may fluctuate with activity in chronic cases.

Limited Range of Motion: Difficulty bending or straightening the knee fully, often accompanied by a sensation of tightness.

Locking Sensation: The knee may temporarily feel stuck in one position.

Catching Feeling: Movements may feel jerky or interrupted by a catching sensation.

Instability: The knee might feel like it could give way, especially during weight-bearing activities.

Popping at the Time of Injury: A popping sound or sensation is common with acute tears.

Grinding or Clicking: Recurrent sounds during movement, especially when the cartilage rubs against joint structures.

Walking Challenges: You might feel discomfort or pain when trying to bear weight on the affected leg.

Activity-Induced Pain: Walking or standing for long periods may exacerbate the pain or make the knee feel unstable.

Meniscus Tear Treatment Options

If you are experiencing common meniscus tear symptoms, especially in combination or with significant pain, swelling, or instability, consult an orthopedic specialist. Early diagnosis and treatment can prevent further damage and improve outcomes. Treatment depends on the tear's size, location, and severity, as well as the patient's age and activity level. Options include:

  • Conservative Management: Rest, ice, compression, and elevation (RICE), along with physical therapy, can be effective for minor tears.
  • Medications: Nonsteroidal anti-inflammatory drugs (NSAIDs) may help reduce pain and swelling.
  • Surgical Intervention: In cases where conservative treatments are insufficient, arthroscopic surgery may be recommended to repair or remove the damaged meniscal tissue.
