Gastrectomy: Stomach Removal Surgery
Our Lee Health Surgical Oncology team offers gastrectomy, a life-saving procedure for individuals at high risk of or diagnosed with stomach cancer. Gastrectomy may also be performed for certain non-cancerous medical conditions, providing patients with renewed health and peace of mind.
Gastrectomy is the surgical removal of the stomach. For some individuals, such as those with Hereditary Diffuse Gastric Cancer (HDGC) caused by a CDH1 gene mutation, this surgery can be a preventive measure to reduce the risk of developing this aggressive cancer. HDGC is particularly challenging to detect early because cancer cells are diffusely scattered throughout the stomach.
The Gastrectomy Procedure
- Preventive Surgery (Prophylactic Total Gastrectomy): Recommended for individuals with the CDH1 gene mutation to prevent the development of HDGC.
- Treatment for Stomach Cancer: Total gastrectomy is often part of the treatment plan for patients with other forms of stomach cancer.
- Non-Cancerous Conditions: In some cases, gastrectomy may be necessary to treat non-cancer-related medical conditions.
Reconstruction After Gastrectomy
After the stomach is removed, the digestive system is reconstructed using a procedure known as Roux-en-Y reconstruction. This ensures that patients can continue to digest food and maintain proper nutrition after surgery.
The Surgery
- Gastrectomy typically takes 4-5 hours, followed by a hospital stay of 7-10 days. After surgery:
- Food and drink may be withheld for 3-5 days to allow healing.
- An X-ray test ensures the surgical connections are secure before resuming eating and drinking.
- Recovery during the first 2-4 weeks can be challenging, with possible discomfort while eating.
- A temporary feeding tube may be inserted to provide nutritional support.
Life After Surgery
Adjusting to life without a stomach takes time and effort. The body adapts to the changes, and most patients achieve a “new normal” over time. Key aspects of recovery include:
- Dietary Adjustments: Eating 6-8 small meals daily, avoiding high-sugar foods, and drinking fluids separately from meals.
- Managing Symptoms: Symptoms such as nausea, heartburn, and dumping syndrome (rapid food movement into the small intestine) are common but manageable through dietary changes and portion control.
- Nutrition and Supplements: Monthly B12 shots, vitamin supplements, and a nutrient-rich diet are essential for long-term health.
