Proxy Access to MyChart

MyChart has the ability for you to view and manage your family member’s medical information. This is called “Proxy access” and allows an adult to log into their personal MyChart account and then connect to other charts.

You can gain access to do this in different ways depending on the type of access you are looking for. What are you looking to do?

Allow another adult to have access to your record

Please log into MyChart and fill out the Grant an Adult Access form within the "Family Member Access" section (Look under the "Settings & Help" Menu).

Gain access to your child (under 12 years old)

Please log into MyChart and fill out the Request Minor Access form within the "Family Member Access" section (Look under the "Settings & Help" Menu).

Gain access to your child (age 12-17)

Due to state and federal privacy laws protecting minor privacy, proxy access for teens (age12-17) must be set up with both the teen and parent in person at your providers office.
