Skilled Nursing Centers

Our Skilled Nursing Centers provide rehabilitative care for patients who, following a hospital stay due to an illness or injury, are not physically ready to return home or to a long-term care facility. Our short-term care centers combine focused medical care with personalized support services to help patients achieve their optimal health and independence.

Our Medicare-certified program provides around-the-clock registered nurse duty and 24-hour physician availability. During their stay, patients may receive:

  • physical therapy
  • occupational therapy
  • speech therapy
  • other restorative care
  • daily recreational therapy
  • services of a social worker
  • assistance from a case manager for discharge planning

Whether recovering from joint/orthopedic surgery, a stroke, heart surgery or chronic obstructive pulmonary disease (COPD), our goal is to assist each patient with his or her rehabilitation needs and prepare him or her to return home as quickly as possible. We are committed to providing the highest level of patient care through personalized rehabilitative treatments.

Skilled Nursing Care

Our staff is dedicated to providing quality care that focuses on the dignity and individual needs of each patient. Nurses help patients learn to care for themselves or teach family caregivers the home-care skills they will need for patients who cannot care for themselves. Other skilled nursing services include:

  • IV therapy, including care of central venous catheters
  • hyperalimentation (nutriennts by IV feeding)
  • tracheostomy care
  • pain management with IM and/or IV narcotics for chronically or terminally ill patients
  • initial application and regulation of oxygen therapy and respiratory monitoring recently-initiated gastrostomy tube feedings
  • wound care
  • treatment of recently diagnosed problems that may result in having an ostomy, draining wounds or loss of control of bowel or bladder function
  • teaching the patient and family skills needed for transition to home setting

Patient Eligibility

Our Skilled Nursing Centers serves patients who require daily skilled rehabilitation therapy and nursing care under the supervision of a physician. Other requirements for admittance include:

Patient Must

  • meet Medicare skilled criteria for admission
  • be at least 18 years of age
  • have no primary psychiatric diagnosis
  • be covered by Medicare or private insurance, or have verification of ability to self-pay (a deposit may be required)
  • be attended by a physician on staff at Lee Health (a physician assignment can be made, if necessary)

Your Care Transition Plan

Soon after admission we assemble an interdisciplinary team of professionals to define patient goals and discuss any potential barriers to progress. A Care Transition Plan is created for each patient and customized to ensure that he or she will return home as quickly and safely as possible.

As the date of discharge approaches and if appropriate, our team can complete a home evaluation to ensure your safe transition to the next level of care.

Most patients who recover at one of our Skilled Nursing Centers are able to be restored to their previous quality of life. The outcomes they receive enable them to return home and enjoy better mobility, less pain, greater joint flexibility and faster recovery.

HealthPark Care & Rehabilitation

HealthPark Care and Rehabilitation is a 112-bed skilled nursing facility located on the beautifully landscaped grounds of HealthPark Florida. Convenient to Cape Coral, Sanibel, Fort Myers Beach, Bonita Springs and all of Lee County, HealthPark Care and Rehabilitation is adjacent to HealthPark Medical Center ensuring the best emergency medical care 24 hours a day.

HealthPark Care & Rehabilitation Center offers a comfortable, home-like environment and features private and semi-private rooms. Newly-renovated, our facility features a restaurant-style dining room, which promotes an atmosphere for social interaction; as well as a café with televisions and two therapy gyms that are open seven days a week.

Lee Memorial Hospital

The Skilled Nursing unit at Lee Memorial Hospital is a newly fashioned 18 bed unit located on the 3rd floor of our downtown hospital. The unique setting of our Skilled Nursing Unit allows for seamless patient transitions while also providing the amenities, services and comfort of the hospital. The opportunity to provide our proven core programming and unique care team approach allows our exceptional staff to focus on providing our patients the right care in the right setting.

Our unit features a friendly dining room, which promotes an atmosphere for social interaction; as well as a welcoming therapy gym that is open seven days a week.

Gulf Coast Medical Center

Gulf Coast Medical Center Skilled Nursing Unit is a 75 private-bed unit rehabilitation facility located on Plantation Road near Gulf Coast Medical Center. Our Skilled Nursing Unit introduces a model for rehabilitative care that allows for seamless patient transitions while also providing the amenities, services, and comfort that foster fulfilling relationships with patients. Each room is private, and each floor has its own dining area.

As part of Lee Health, Skilled Nursing works collaboratively with other departments and programs to ensure the best and safest care, especially upon discharge.

Care Transitions

The Care Transitions Program involves an in-home visit from a transition coach who looks for safety issues, reviews medications, nutrition needs and monitors medications. The transition coach also encourages the patient to set personal health goals by developing an action plan with the patient and his or her caregiver.

Home Health Services

Home Health offers a range of services to meet the special medical, physical and emotional needs of patients. The team of health care professionals—which includes registered nurses (RNs); physical, speech and occupational therapists; home health aides and medical social workers—develop a comprehensive home care plan, monitor and review each patients’ progress.

Using advanced technology, Home Health Services offers patients the option of enrolling in the Telehealth program, which monitors vital signs daily from each patient’s home and transmits the information to a central monitoring station where it is reviewed by registered nurses. The Telehealth nurses assess the results and communicate the status and any changes to the patient’s doctor.

Our Expert Staff

Our staff at the Skilled Nursing Unit is comprised of a team of registered nurses (RNs), certified nursing assistants (CNAs), physical therapists, occupational therapists and speech therapists. These specialized therapists excel in multiple areas, such as balance management, stroke and cardiac rehabilitation. A knowledgeable social worker is also part of the care team that will assist with your continued success throughout your recovery.

We also employ registered dietitians, who work collaboratively with clinical and therapy teams to develop menus and specialized diets.

Our team utilizes the latest clinical and rehabilitative treatments as they work to create a patient-centered plan to ensure your successful transition to the next level in care.
